DEBARKATION AUTHORIZATION FOR MINORS

Our onboard Security system prohibits children under the age of 18 from disembarking the ship unless accompanied by

an adult in their same stateroom. If your party occupies more than one stateroom, please take a moment to review and

complete this form authorizing any additional adults in your party to accompany your child ashore. This also applies to

parents and guardians occupying a room separate from their children. This form may also be used to authorize

children under the age of 18 to go ashore unaccompanied by an adult. Completed forms should be returned to the

Guest Services Desk, located on Deck 3, Midship.

ACCOMPANIED ASHORE BY AUTHORIZED ADULTS

I represent that I am the parent or guardian of the child/children listed below who are under the age of 18 and hereby

authorize the listed children to debark the ship accompanied by the listed adults who are age 18 or above. I accept full

responsibility for the actions of the children while ashore and release and hold Disney Cruise Line harmless from any and

all responsibility or liability arising from the listed children being accompanied by the adults as well as the acts of the

children while ashore.

Please note: Unaccompanied minors require a Key to the World card and original government issued photo ID when

going ashore in all ports except Disney Castaway Cay, where only a Key to the World card is required.
