Specification
Documents presented as proof of identity and date of birth
must be original or certified copy from the issuing agency.
You must present at least one document that contains your
full current name and date of birth.
If the document does not contain your current legal name,
you must present proof of legal name change.
Proof of residence address must include your first and last
name and current residence address.

of the following to prove identity and date of
birth. The document you present must include both your name and
date of birth. If the document does not contain both your name and
date of birth you must present an additional document.
Birth certificate issued by a U.S. Territorial government, the
District of Columbia, or the government of a state or political
subdivision of the U.S.(Hospital -issued birth certificate, hospital
registration card, or birth registration or baptismal certificate are
not acceptable).
A Canadian birth certificate.
U.S. Consular Report of Birth Abroad (FS-240)
U.S. Government-issued Certification of Report of Birth (DS-
1350 or FS-545)
Request for Verification of Birth (DD372)
U.S. Passport, not expired more than five years
U.S. Passport Card, not expired more than five years
U.S. Territory Passport, not expired more than five years
Tribal ID Card from a federally recognized tribe located in
Oregon, or with an Oregon affiliation, approved by DMV
Foreign Passport containing English or with English translation,
not expired more than five years
Certificate of Citizenship (N560 and N561)
Certificate of Naturalization (N550, N570, and N578)
U.S. immigration or naturalization documents, not expired
more than five years, including:
o U.S. Citizen Identification Card (I179 and I197)
o Resident Alien Card or Permanent Resident Card (I551)

Out-of-state, District of Columbia, U.S. Territorial government or
Canadian driver license, instruction permit, or ID card, that
contains the applicant’s photograph, not expired more than
one year. Cards that are hole-punched or marked “Not Valid as
ID” are not acceptable.
Oregon driver license, instruction permit, or ID card not expired
more than thirteen years (must have a photograph)
Non-immigrant visa issued by the U.S. Department of State, not
expired more than five years
Temporary immigrant visa (I551) issued by the U.S. Department
of State, not expired more than five years
U.S. Department of State driver license or non-driver ID card not
expired more than five years
Oregon Concealed Weapon Permit/Concealed Handgun
License, not expired more than five years
Social Security Card or Individual Taxpayer Identification
Number issuance letter from the IRS
A Mexican Consulate ID Card, not expired more than five

A letter verifying identity provided by an Oregon County
Community Corrections agency, approved by Oregon DMV
A letter verifying identity provided by the U.S. Pretrial Services,

A letter verifying identity provided by the U.S. District Court
Probation Office, approved by Oregon DMV
Oregon Department of Correction Release Identification Card,

If you are unable to present one document from above you will
need to present at least two documents from this list. At least one
of the two required documents must contain both your full current
name and date of birth.
U.S. Military discharge papers
An Oregon student body card
U.S. or foreign certified school records
An Oregon Job Corps Identification Card
A W2 or 1099 tax form for the current tax year
A Medicare Card or Medicaid Card
A court document issued by a court in the U.S. that shows the
applicant is a party to the judicial proceeding. The document
must contain court signatures and seals. Acceptable documents
are:
o Gender, name change, or both
o Adoption, guardianship, custody, or child support
o Judgement of dissolution or annulment of marriage or
domestic partnership
o A court document issued by a Bankruptcy court that
shows the applicant is a party to a bankruptcy proceeding
o
U.S. government issued marriage license, domestic

partnership, or Certificate of Record of Marriage
Border Crossing Card (DSP150, I185, I186 and I586)
Foreign government issued birth certificate, marriage license,
driver license, military ID card, passport (expired more than five
years) or voter card
Social Security Card transmittal document
A benefits letter or end of year benefit statement (Form SSA-
1099) issued by the Social Security Administration
Veterans’ Universal Access Identification Card
A letter issued by the Oregon Health Plan, if dated within the last
year
Merchant Marine Identification Card
U.S. Tribal Identification Card
Identification Card that contain photograph of the applicant
issued by a government within the U.S. or the U.S. government
Proof of Address
You are required to present at least on

Acceptable proof includes:
Any proof of identity and date of birth document listed above that
includes applicant name and current residence address.
Mortgage documents or a rental or lease agreement.
Monthly rental receipt or letter from a hotel, motel, campground
or recreational vehicle park that permits extended stays.
A verbal statement from any person residing at the same
residence address as provided by the applicant to DMV. The
person making the verbal statement must accompany the
applicant at the time of application and present one acceptable
proof of address document as set forth in rule.
Utility hook-up order or account statement issued by the service
provider.
A loan agreement, payment booklet, voucher, statement or other
financial record.
Any document delivered to the current resident address by the
United States Postal Service, FedEx, UPS, or other delivery
service. DMV will accept a document with or without the
envelope or packaging as long as the residence address is
printed on the document.
Oregon vehicle title or registration documents belonging to the
primary owner listed and containing the owner's residence
address.
Oregon manufactured structure ownership documents.
Oregon voter notification card.
Selective Service card.
Insurance card, insurance policy, medical records or health
benefits card.
Educational institution transcript forms or other school
documents showing enrollment for the current school year.
An unexpired professional license issued by an agency in the
United States.
Form DS2019, Certificate of Eligibility for Exchange Visitor (J-1)
Status.
A pay stub, W2, 1099 or income tax return.
Letter from the United States Postal Service, Oregon State

Hospital, Oregon Veteran Rehabilitation Center, a homeless
shelter, a transitional service provider, nursing home,
assisted/independent living care facility/home, adult care service
provider/skill nursing facility, or halfway/group home certifying
the applicant's residence address, approved by DMV.
A letter from a corporation or business that provides corporate
housing to employees.
Proof of Full Legal
Name Change
If your proof of identity and date of birth does not contain your current
legal name, you are required to present an acceptable document as
proof of legal name change. Acceptable proof includes:
U.S. or foreign government-issued marriage license/certificate
Death certificate of spouse
Divorce decree
Certificate of domestic partnership
A judgment of dissolution or annulment of marriage or domestic
partnership
An adoption decree
Court order or judgment legal name change

To apply for an ID card at a DMV office the applicant will need an
appointment. Applicants or representative can schedule an appointment
online at dmv2u.oregon.gov.
