CONSENT TO PASSPORT
APPLICATION FOR A MINOR
Parents must give their consent in writing for passports issued to a child under the age of 18.
If parents have joint custody, both parents have to consent to the passport application. At least one of them must
accompany the child to the appointment AND in addition produce valid passports for both parents as well as the
child’s passport. If only one parent is accompanying the minor to the appointment, the signature of the absent
parent on this form must be notarized.
If one of the parents have sole custody, proof of sole custody for the child (awarded by a court order or
agreement) must be presented.
