INSTRUCTIONS FOR COMPLETING A RESUME FOR LICENSING

YOUR resume should document a complete employment history. However, for licensing purposes, the resume

must provide a detailed description of the social work practice associated with employment sites and time

frames found on the Supervision Verification Form(s).

F. Certified Social Worker.

(6) Social work experience required to obtain a certified social worker license may include one or more of the

following:

(a) Case management;

(b) Information and referral;

(d) Research:

(e) Community organization;

(f) Administrative activities; and

(g) Supervision.

Your resume should document a complete employment history. However, for licensing purposes, the resume

must document and describe clinical social work experience associated with employment sites and time

frames found on the Supervision Verification Form(s).

.02 Definitions.

(7) "Clinical social work experience" means:

(a) Completing assessments;

(b) Formulating diagnostic impressions or a diagnosis;

(d) Treating behavioral health disorders, including but not limited to substance use disorders, addictive

disorders and other conditions; and

(e) Providing psychotherapy.

G. Certified Social Worker-Clinical.

(7) Clinical social work experience for the purpose of obtaining a certified social worker-clinical license shall

be in direct service to clients and may not consist solely of:

(a) Telephonic clinical assessment;

(b) Triage;

(d) Discharge planning.

(9) Half of the supervised clinical social work experience required to obtain a certified social worker-clinical

license shall consist of face-to-face client contact.
