FOREWORD
This publication presents state and project immunization policies that were validated in
February 2002. It reflects state laws, regulations, or rule making that impose vaccination
requirements for enrollment and/or attendance in day care, Head Start, kindergarten, grades 1
through 12, colleges and universities. Also reported are policies for pre-marital and hospital
employee screening for rubella susceptibility and the screening of pregnant women for Hepatitis
B surface antigen positivity and the reporting of same.
The policies are vaccine specific and catalogued by state and are presented in both table and
map formats. Because there are many state variances in requirements, narrative descriptions
referencing age and dose requirements are also included.
This information represents the collaborative efforts of immunization program managers and
staff in the 50 states, the District of Columbia, and the Commonwealth of Puerto Rico and staff
from the Centers for Disease Control and Prevention's National Immunization Program.
